Models of Service
Each service area has its own unique service model which is research based and promotes collaborative working between multi-disciplinary agencies, the client, family, advocates and others involved in the persons care. Assessments & Care Planning
We endeavor to ensure that each client has a regular, full and comprehensive assessment in order to identify and respond to individual need and to ensure that care plans are individualised, promote a person centered approach and provide a basis for each individual to meet their own potential. We believe in an holistic approach to care where physical, psychological and social needs are recognised as having equal importance. We are committed to ensuring the full participation of clients and relatives or advocates in the care planning process.
Outcomes & Service Review
We endeavor to ensure that practice is measurable to ensure its appropriateness and to give a measure of success. We continually try to improve the service we deliver to achieve the best possible outcomes. At Positive Lifestyles our Senior Managers carry out monthly audits within our services, this enables us to monitor standards of care and improve on the care we deliver, in addition to offering specialist advice to staff teams on how to continually improve service provision.
Research Based Practice
We endeavor to ensure that care practices are research based and that staff are trained appropriately to gain a knowledge of contemporary research findings. We encourage openness and honesty, by doing this we are able to acknowledge any weaknesses and learn lessons from experiences. We take any complaint seriously and ensure we reply to the same with openness and honesty.
Our homes and services are inspected and regulated by the Care Standards Inspectorate Wales (CSIW), in accordance with the Care Standards Act 2000
