Continuing Health & Nursing Care
Over the next twelve months we are working in partnership with local authorities to develop residential services for those individuals who will require 'Continuing Health Care' on discharge from hospital and acute settings.
The locality of our homes will allow for easy access to local community facilities, enabling clients to be present and actively participate in their local community.
Our services will provide residential accommodation for varying lengths of stay; long-term, short-term, respite and periods of assessment are catered for.
Each client will have the comfort of their own bed-sitting room with en-suite facilities, complete with appropriate furnishing- together with a shared lounge, dining room, visitors room and activities room.
Who will we support ?
Continuing Health Services will supports adults (18-64yrs) with:
- Learning Disabilities
- Aspergers and Autistic Spectrum Disorders
- Mild to Severe Challenging Behaviour
- Mental Health Disorders
- Personality Disorders
We will focus on individuals:
- Making the transition from children services
- Requiring after-care on hospital discharge
- Requiring supervised discharge under MHA
- Requiring support under Guardianship Orders
What services will we provide?
At Positive Lifestyles we recognise that individuals with learning disabilities and/or mental health concerns may require a combination of specialist support to develop the necessary skills to improve their chances of independence and continuity of life in the local community. The following services will be provided at out Continuing Health Units:

- Assessment, Care Planning and Treatment
- Positive Behaviour Management
- Cogitative Behaviour Therapy
- Consultant Psychiatry
- Psychology
- Speech Therapy
- Vocational and Occupational Therapy
- Physiotherapy
Our services will maintain a complement of highly committed staff experienced, in listening, understanding and responding to clients needs, and actively supporting the therapeutic approach of the unit. Both units aim to ensure that each client received a highly professional and client centred service; ensuring treatment is provided by experience and dedicated multi-disciplinary teams, within a safe and homely environment. A tailored package of care is developed to meet each clients needs, which incorporates risk assessment, community presence, clinical, personal, social and educational needs.
