Referrals
We welcome referrals from private clients and their families, local authorities, local and health services.
The appropriate case manager for each prospective client must complete a referral form. The information received on the referral form will determine whether or not the prospective client meets the admission criteria for a proposed home.

Assessment
Following a referral for placement, a senior clinician, in liaison with the `all persons` involved in the clients care, will carry out a pre- assessment of the clients needs.
Information detailed in the assessment will be gathered using the following format:
- Direct observations - discussions with client
- Written report and discussion with clients Case Manager
- Extracting information from previous care notes and reports.
- Extracting information from previous behavioral reports and analysis (if appropriate)
- Structured interviews with current carers and family members.
- Structured interviews with professional and specialist involvement
Fees
The pre-assessment will determine the levels of support required and the fees payable to ensure the clients comprehensive needs are met during their stay.
Positive Lifestyles does not charge to undertake pre-assessments within the South Wales area. For information and charges for a pre-assessment outside of South Wales region please contact us.
Contract
All contractual, financial matters and appropriate individual care contracts must be agreed and signed prior to a client moving into one of our care homes.
