Registered Care Homes
Our Care homes are located in the Cardiff and Vale of Glamorgan area of South Wales. In line with best practice Positive Lifestyles offers Registered Care Homes accommodation in small domestic style homes, providing personal care in a residential setting.
Clients have the comfort of their own bed-sitting room, all bedrooms fully furnished with; a bed and soft furnishings, fitted wardrobes, chest of drawers, bed-side table and lamp and two armchairs. All rooms are well heated and ventilated and can be controlled by the individual within the rooms. Individuals are encouraged to bring their own furnishings at their request.
Who do we support ?
Our Care Homes support Young Adults (18-64yrs) and Young People (16-17yrs) with:
- Learning Disabilities
- Aspergers & Autistic Spectrum Disorders
- Mild to Severe Challenging Behavior
- Mental Health Disorders
- Dual Diagnosis
- Complex Care Needs
- Challenging Behavior
Our Care Homes also support individuals:
- Making the transition from children services
- Requiring after-care on hospital discharge
- Requiring supervised discharge under MHA
- Requiring support under Guardianship Orders
What services do we provide?
At Positive Lifestyles we recognise that individuals with learning disabilities and/or mental health concerns may require a combination of specialist support to develop the necessary skills to improve their chances of independence and continuity of life in the local community. The following services are provided at our Care Homes:
- Personal Care & Supervision
- Assessment & Care Planning
- Positive Behavior Management
- Cognitive Behavior Therapy
- Transitional Planning
- Vocational Services
Nursing Care is not provided within these homes, where the individual requires such services, they will be provided by the Generic Nursing and Medical Services or our Continuing Health Services.
