Providing successful outcomes for adults who may challenge existing service provision is the tracscare mission. In diagnosis specific homes, our specialist services can provide care for challenging adults who experience difficulties in finding placements.

tracscare is a specialist provider of care and rehabilitation, with over 25 years experience of individual care packages for clients with challenging or complex needs. Individual care and support is provided through our dedicated Nurse Led Therapy Centre's, Residential Care Homes and Supported Living accommodation.

tracscare Care Pathways offer a continuum of care to improve the lives of individuals who may have:

tracscare provides an individual service based upon assessed needs, with a flexible care response, which may vary from the provision of a Home for life, to the focused rehabilitation of preparing clients for more independent living. Individuals care is tailored to their needs, and based on an individual assessment upon entry.

Tracscare are experienced at providing positive results for clients with 62% of clients remaining stable or progressing during 2010-11.
